Minnesota State 4-H Dog Project
2009 State 4-H Dog Show
Registration Instructions for Extension Regional Offices

Prior to your County Fair/Qualifying Dog Show:
  1. Print out the 2009 Show Premium List and all 2009 Show Forms.
  2. Duplicate the 2009 Show Premium List and all 2009 Show Forms so that you can distribute one packet to each 4-Her eligible for the State 4-H Dog Show.
  3. Set a county deadline for your 4-Hers to return all applicable 2009 Show Forms and fees to your county extension office.  Note that checks are made payable to University of MN.
Prior to arrival at the 4-H Building for State Dog Show Registration Processing:
  1. Be sure each Member Entry Form has stapled to it a photocopy of the Minnesota 4-H Dog Project Identification Form/Vaccination Certificate and a copy of the MN 4-H Member Information Form (either from the Member Enrollment form or Member Information Authorization Form - whichever the county used).  Additionally, if the Member Entry Form includes registration for any agility class, be sure a photocopy of the 4-H Dog Agility Release Form (found on page 28 of the Minnesota 4-H Dog Agility Rules Revised 2005) is stapled behind the ID Form/Vaccination Certificate.
  2. Alphabetize all Member Entry Forms by the 4-Hers last name.
  3. Alphabetize all Camping Reservation Forms by 4-Hers last name.  (Do NOT staple any Camping Reservation Forms to the Member Entry Forms.)
  4. To speed up the State Dog Show Registration Process, please three-hole punch all forms so that they may be place in a three-ring notebook.
  5. Using the completed Member Entry Forms, enter State Dog Show registrations into the FairPlus! software.  EE's and Regional Office Support Staff will receive instructions from Todd Mehrkens on entering registrations into FairPlus! in August. Check the 4-H Software Support Site for details, http://www.fourh.umn.edu/mgtsoftware/index.html
  6. Verify State Show Checklist for each State 4-H Dog Show Participant
